Processing of personal data and health information

I agree to the processing and storage of my personal data (first name, last name, phone number, email address) for the purposes of providing services, managing bookings, and maintaining customer loyalty records.

I also agree to the processing of the health information provided by me for the purpose of assessing the safety and suitability of the service. Health data is used solely for providing the service and ensuring its safety.

Submission of health information

The client is required to provide, during the booking process, all relevant, complete, and accurate health information necessary for the provision of the service, using the designated Health Information field.

Important health information includes, among other things, allergies, chronic conditions, pregnancy, skin sensitivity, injuries, and any other circumstances that may affect the safety or suitability of the service.

The service provider has the right to assess the suitability of the service based on the health information provided by the client and, if necessary, to refuse to provide the service or adjust it in the interest of the client’s health condition.

Liability

If the client fails to provide important health information or provides incorrect or incomplete information, the service provider shall not be liable for any damage or health-related consequences to the extent that they result from the omitted or incorrect information.
